Documents required for porting Verizon Numbers

List of required documents (📜):

1. Unlock confirmation screenshot

Before we can begin the porting process, your Verizon number must be unlocked for transfer.
Please share a screenshot that confirms your number is unlocked.

How to unlock your Verizon number:

  • Log in to your Verizon account via the web or My Verizon app.

  • Navigate to Account → My Devices → Manage Device → Device Unlock.

  • If the number is locked, select Unlock and follow the instructions.

  • Once unlocked, take a screenshot of the confirmation page showing the number and unlock status.

2. Letter of Authorization (LOA) dated within the last 30 days.

  • LOA must be dated within the last 30 days.

  • Download the form, fill in all required details, and sign it.

  • Ensure that the information matches Verizon’s on-file records exactly.

  • Submit the completed form along with the other required documents.


3. Screenshot of Verizon Account Details

Capture a screenshot of your Verizon account page that clearly shows:

  • The Verizon phone number you want to port.

  • The Account Holder Name.

  • The Billing Address on record.

  • Please provide the Account Number for the current provider


4. Porting PIN

The Porting PIN is mandatory during the porting process.

How to get your Verizon Porting PIN:

  • Log in to My Verizon → Account → Number Transfer PIN.

  • Take note of the PIN provided (valid for 7 days).


⚠️ Important Notes:

  • There is no downtime—the number remains active until it is ported to ServiceAgent.

  • Porting usually takes 2–3 weeks for US and Canadian numbers, depending on Verizon’s processing time.

  • Do NOT cancel your Verizon account until the porting process is fully completed.

Additional Comments

Customer Name

The Customer Name is the name of the admin or the owner, and it should appear exactly as it does on the on-file records of the current carrier or the CSR (if the service is under the name of an individual).

Business Name

Name of the business as mentioned on on-file records of the current carrier or the CSR (If the service is in your company's name).

Service Address

Please ask your current/losing carrier to provide a Customer Service Record and enter the Service Address mentioned on it. 

 

⚠️Incorrect information will lead to rejection

Account Number

Please ask your losing carrier if the numbers you are porting have an Account Number associated with them and provide the same if applicable 

⚠️Missing information will lead to rejection

PIN

Please ask your losing carrier if the numbers you are porting have a Port-Out PIN associated with them and provide the same if applicable.

⚠️Missing information will lead to rejection

  


Pro Tips (💡):

  • Request your Customer Service Record (CSR) from Verizon to ensure all details match.

  • LOA must include your actual U.S. address associated with the number.
